pop3 works, imap and SWA don't see mailbox?

We've been having the same problem repeatedly.

In two cases, I assume it was caused by an aborting client or IMAP connection while
a new folder was being created.
In the latest case, I don't know what exactly caused the failure.
According to the user, he was just trying to empty his wastebasket.

Could anyone comment on this issue?
Steps to fix this?
Ways to avoid this in the future?

So far, using omcpoutu; omdelu and omcpinu on the affected mailbox fixed the problem,
but of course this loses some emails and leaves the users without access to their emails for quite some time...

Just one more thing:
In this latest instance, I noticed that one of the user's IMAP processes was still running on the server. After killing it, the user could at least log in to SWA again.

I ran "omshowlog -p 60 -l 99" as recommended in a previous message in this thread;
the output can be provided on request.

Before, logging on to SWA always failed; the catalina.out log contained the following message:
