Out of Office sent on wrong users behalf [Solved]

When one of my users receives an outlook calendar invitation, the person sending the invitation gets another users out of office reply (currently enabled). This has happened twice now (over a 2 week period). Same sender, receiver and out of office reply.
I have no idea where to start for troubleshooting this. Any ideas?
